One-Sided Stabbing Eye Pain? It Might Not Be a Migraine

While many patients are told they have migraines or sinus infections, these specific, one-sided 'stabbing' pains often point to a group of conditions called Trigeminal Autonomic Cephalalgias (TACs).

TACs are a group of primary headache disorders characterized by intense pain on one side of the head (unilateral) and "autonomic" symptoms—physical reactions in the face that happen automatically during an attack such as tearing, congestion, droopy eyelid, facial fullness and others.

The most well-known member of this family is Cluster Headache, but there are several others, including Hemicrania Continua, Paroxysmal Hemicrania and SUNHA (Short-lasting Unilateral Neuralgiform Headache Attacks).

Common Symptoms: What to Look For

The hallmark of a TAC is that the pain is almost always strictly on one side. During an attack, you may notice one or more of the following on the same side as the pain:

  • Eye Changes: Redness (bloodshot eye), tearing, or a drooping eyelid (ptosis).

  • Nasal Congestion: A runny or "stuffy" nose only on the side of the headache.

  • Facial Sweating: Noticeable perspiration on the forehead or cheek.

  • Restlessness: Unlike migraine patients who often want to lie in a dark, quiet room, people experiencing a TAC attack often feel the need to pace or move around.

Because these symptoms involve the eyes and nose, many patients spend months (or years) treating what they think is a "sinus infection" or an "allergy issue."
