Book Appointment

Please keep the following in mind:
- The initial consultation occurs via the à la carte plan.
- At the end of your consultation, we will decide together whether a membership or continued à la carte services would serve you best.
- Please note that the initial consultation fee will be charged at the start of the visit and will be applied to whichever option you choose.
- The consultation link will ask for your basic information, to sign policy forms, and enter payment information
- You will subsequently be given a code to use while downloading the SigmaMD patient app (see FAQ for more information)
