    • Standard Forms: Completion of school, work, and sports clearance forms.

    • If you suffer from chronic migraines or recurring neurological symptoms, 4 visits a year plus procedures in a traditional system can easily cost $1,500 - $2,500 out-of-pocket before your deductible is met.

    • Our membership provides superior care and peace of mind for a predictable monthly cost.

    • Neurological conditions like chronic migraine or persistent symptoms after concussion require consistent, long-term management to see results.

    • To ensure the highest quality of care and clinical outcomes, our memberships are designed as a 12-month therapeutic partnership.

    • This allows Dr. Patel to manage your care holistically without the interruptions of traditional insurance-based timelines.

Please note: Membership fees are non-refundable. If you choose a monthly payment plan, the remaining balance of the annual term is due upon early cancellation.

Botox Injections
Included - does not include medication

  • Botox (onabotulinumtoxinA) is an FDA-approved preventive treatment for chronic migraine that works by blocking the release of neurotransmitters that carry pain signals to your brain. By injecting tiny amounts into specific muscles around the head and neck, it effectively "paralyzes" the nerve endings to prevent pain pathways from activating before a headache even starts. Additionally, when applied to the jaw muscles, Botox provides significant relief for TMJ by relaxing the tension and grinding that often trigger facial pain.

Nerve Blocks/Trigger Point
Included - max once per 4 weeks

  • A nerve block for headaches is a quick, outpatient procedure where a medical professional injects a small amount of local anesthetic (sometimes combined with a steroid) near specific nerves in the head or neck to "shut off" pain signals and provide quick relief.

Trigger point injections are used to treat painful "knots" in muscles that won't relax. A clinician injects a local anesthetic (and sometimes a steroid) directly into the sensitive point.

Toradol Injections
Included - max 2 per month

  • A Toradol (ketorolac) injection is a powerful, non-opioid medication often used as a "rescue" treatment for severe migraines or headaches that haven't responded to standard over-the-counter drugs or other abortive medication.

SPG Blocks
Included - max once per 4 weeks

  • An SPG (Sphenopalatine Ganglion) block is a minimally invasive treatment that targets a specific cluster of nerves located deep behind the nose. This "nerve junction" is a major relay station for pain signals and autonomic symptoms (like watery eyes or congestion) associated with severe headaches. This can also be helpful for treating facial pain.
