Initial Consultation: All new patient journeys begin with an in-depth, 60-minute initial consultation via our À La Carte plan.
Flexible Planning: At the end of your visit, Dr. Patel will work with you to decide if a monthly Membership or continued À La Carte visits best suit your needs.
Applied Fees: Your consultation fee is charged at the start of the visit and will be applied toward whichever care path you choose.
Easy Registration: The booking link will prompt you for basic info, policy signatures, and payment info. Afterward, you’ll receive a code to download the SigmaMD patient app for communication with our office. This is our concierge patient app, where you can message Dr. Patel directly, view your chart, book appointments and more!
Your Clear Path to Care
However, because we operate under a direct-care model, our relationship with Medicare is unique.
Dr. Patel has formally opted out of the federal Medicare program. This allows us to provide deeply personalized, unhurried specialized neurological care without the restrictions and time constraints imposed by government insurance bureaucracies.
What this means for you:
Direct Payment: Medicare patients pay our standard, transparent office fees directly out-of-pocket at the time of service.
No Reimbursement: Under federal law, neither the clinic nor you (the patient) can submit claims to Medicare or any supplemental Medigap policies for reimbursement. Our services are strictly private-pay.
Written Agreement Required: Before your first appointment, federal regulations require that we both sign a standard, straightforward Medicare Private Contract acknowledging this direct-payment arrangement.
The Good News: Your Medicare benefits still work outside our doors! Even though you cannot be reimbursed for your face-to-face time with Dr. Patel, your traditional Medicare benefits remain fully active and unaffected for everything else. Because Dr. Patel maintains valid federal ordering and referring credentials, you can continue to use your regular Medicare coverage for:
Specialized imaging (such as MRIs or CT scans) ordered by our clinic.
Outpatient laboratory bloodwork.
Prescription medications at your local or mail-order pharmacy.
Referrals to physical therapy or other participating medical specialists.
